Our forms have been revised as of January 12, 2009. The new forms are on the website. Applications started before January 12, 2009, may use the old forms, but must be submitted before July 1, 2009. "Started before January 12, 2009" means your Control Evaluation must be dated on or before January 12, 2009. Applications started after January 12, 2009 must be on the new forms. All applications for membership submitted after June 30, 2009 must be on the new forms. If you do not use the correct (new) forms, your application package will be returned to you.
Many applications are returned because they are incomplete or incorrectly completed. If you have any questions, contact us.
If you are making an application for a pet other than a dog, contact us before sending in your application. There are a lot of animals our insurance will not cover.

The Love on a Leash Brochure is a double-sided, tri-fold brochure. You should print it on both sides of the paper and fold it in thirds.
